Follow below steps to remove errors:
1. Change driver value:
String driver = "com.mysql.jdbc.Driver";

2. Then add the the mysql connector jar named:
"mysql-connector-java-5.0.4-bin.jar"

3. keep the jsp name "BookEntryForm.jsp"
